TB Survey for Scottish Physicians January 5, 2012

Dear Colleague,

 Please find enclosed a link to a survey (online survey in Survey MonkeyTM) to evaluate the Tuberculosis surveillance system at your area:

 We would be grateful if you would take the time to complete the survey before Wednesday 18th January 2012.

 Completing this survey takes only approximately 20 minutes, and it is completely anonymous.

 Please do NOT include personal information (e.g. names of individuals, addresses, patient information) or information on private companies in your responses.

 This work is being undertaken to evaluate the English and Scottish tuberculosis surveillance systems, furthermore by contrasting them to help Health Protection Scotland to change to the highest quality of TB surveillance achievable in the practice.

 Health Protection Scotland will collect and analyse survey responses. All survey responses will be deleted once a full dataset is complete; HPS will retain and securely store a copy of the dataset until issue of the final report the latest in September 2012. After request we will send you the report of the results.
